In outpatient rehab, the plan of care prescribes a course of visits — and outcomes depend on the patient actually completing it. Yet patients drop off constantly: they feel better, life gets in the way, a copay stings, and the prescribed twelve visits become six. That's both a clinical gap and a revenue gap, and most clinics never measure it. This dashboard puts attended-versus-prescribed visits on one screen so the drop-off becomes visible and actionable.

What this report answers

The Completion vs Prescription page leads with the POC completion rate, early drop-off rate, and the prescribed and attended visit totals, then breaks completion down by discipline and by therapist and shows prescribed-vs-attended visits by site. The Drop-Off Drivers page surfaces the why and where: active plans of care, episodes completed, early drop-off rate by payer and by diagnosis group, and a completion-rate trend by start month. Together they answer the clinical-operations question: are patients finishing the care we prescribe, and if not, who's dropping and why?

The metrics that matter

POC completion rate — attended over prescribed visits — is the headline, because in rehab the prescribed dosage is the treatment, and a completed plan is both better care and the full revenue of the episode. Early drop-off rate isolates the patients who left well before the course finished, which is the most recoverable group. Breaking both by payer and diagnosis group is what makes them actionable: a high drop-off concentrated in one payer often points to copay or coverage friction, while concentration in a diagnosis group can point to an engagement or expectation-setting gap.

Why the data is trapped across the systems

Prescribed visit counts live in the plan of care, attended visits in the scheduling and documentation modules, and the episode itself spans weeks — all inside Prompt, Raintree, Net Health / Clinicient, WebPT, TheraOffice, Jane, or Fusion. The canned reports rarely express completion as attended-vs-prescribed at the episode level, and almost never break drop-off by payer and diagnosis. Reconstructing the episode and comparing it to the prescription means exporting and modeling the data, which is what this template does.

How to read it

Start with the completion rate, then go to early drop-off rate by payer and diagnosis group to find the recoverable losses, and read completion by therapist for engagement differences worth coaching. A falling completion-rate trend is an early signal the clinic's engagement or scheduling is slipping. The sample uses fully synthetic, anonymized episode data — no PHI — so you can see the finished layout before bringing your own.

Metrics it tracks

Active Plans of CareCount of plan-of-care episodes in the period.
POC Completion RateVisits attended ÷ visits prescribed across all episodes — how much of the prescribed course is actually delivered.
Episodes CompletedEpisodes that finished the prescribed course, via a 1/0 flag.
Early Drop-Off RateEpisodes that dropped well before completion ÷ all episodes, via a 1/0 flag.
Visits PrescribedSum of visits prescribed across all plans of care.

Frequently asked questions

What is plan-of-care completion rate in outpatient rehab?

It's visits attended divided by visits prescribed across plan-of-care episodes — how much of the prescribed course of care patients actually complete. Because rehab outcomes depend on the prescribed dosage, completion rate is both a clinical-quality measure and a revenue measure: an incomplete plan is under-treated care and unrealized episode revenue.

Why do rehab patients drop off before finishing their plan of care?

The common reasons are feeling better early, transportation and scheduling friction, copay or coverage cost, and life events. Breaking early drop-off by payer and diagnosis group separates the recoverable losses (cost or engagement) from the ones tied to a quick recovery, so a clinic knows where outreach or expectation-setting actually helps.

How is this different from a cancellation report?

Cancellations measure individual missed appointments; plan-of-care completion measures the whole episode — whether the patient finished the prescribed course. A patient can have few cancellations but still drop off early, so the two reports answer different questions and are most useful side by side.
